Annual reporting of health care coverage information
As part of our annual outreach, AmeriHealth New Jersey subscribers will be receiving tax forms to assist them with their 2021 tax year filings.
New deadline for 1095‑B forms
In recent years, AmeriHealth New Jersey has mailed 1095‑B forms by the January 31 deadline to applicable subscribers that verifies their health care coverage. This year, the deadline to mail 1095‑B forms to subscribers has been extended, so all 1095‑B forms for the 2021 coverage year will be sent by March 2 instead of January 31.
State mandates for CA, NJ, RI, and the District of Columbia
As a reminder, we will also report 1095‑A and 1095‑B information to New Jersey and 1095‑B information to California, Rhode Island, and the District of Columbia by their respective due dates. Reporting for these entities began in 2020 and 2021 for the 2019 and 2020 tax years.

More about 1095 forms
Please note that there are three versions of 1095 forms: A, B, and C. 1095‑B forms are provided by insurers, while 1095‑A forms are sent by the federal government to purchasers of insurance through the federal marketplace, and 1095‑C forms are sent by self‑funded groups to subscribers in their health plans. Form MA 1099‑HC for Massachusetts residents
This form is mailed to subscribers with a permanent address in Massachusetts only. Similar to the federal 1095‑B form, the MA 1099‑HC form for Massachusetts residents verifies that they had minimum essential health care coverage in 2021 as required by the Commonwealth of Massachusetts. This is issued to subscribers only, unless we receive a request from a subscriber to issue a duplicate form to an enrolled spouse and/or dependent.
What you and your customers need to know about MA 1099‑HC forms
As part of our annual outreach, we mailed customers with employees who reside in Massachusetts a letter last month reminding them about minimum creditable coverage (MCC) requirements under Massachusetts health care reform law. The Commonwealth of Massachusetts requires residents who are 18 or older to have health insurance.
For those customers who submitted a completed attestation form to AmeriHealth New Jersey, we will be issuing MA 1099‑HC: Individual Mandate for Massachusetts Health Care Coverage forms to their eligible subscribers by the January 31 deadline. Please note: If AmeriHealth New Jersey did not receive a completed attestation form from your applicable customers within the requested timeframe, AmeriHealth New Jersey assumes the customer has either opted out of our MA 1099‑HC form process or their plans did not meet the Massachusetts mandate.
If your customer qualifies for the Massachusetts mandate and has opted out of our form process or did not return a completed attestation form, it will be the customer's responsibility to produce the MA 1099‑HC forms for their members for this tax year. The MA 1099‑HC forms include the names and birth dates of covered individuals, the months of coverage for each individual listed, and the name of the issuer.
We ask that subscribers consult their tax advisor and refer to the Commonwealth of Massachusetts website to learn more about MA 1099‑HC and MCC requirements.
